Type the numbers, questions and answers on a plain sheet of paper or download Word document.   Incomplete applications will be returned.  Do not fax or email application.

1. List:  organization name, address, contact person, phone number, email address, website, & Tax ID number.
2. Describe the proposed project and how it came about.  Include the challenges the project addresses.
3. How will the project benefit the community?
4. Who will be affected by this project?
5. Is the task doable by a college student?  Have you provided enough information or experienced staff to help the student accomplish the Project?  To whom will the student report (name & title)?
6. Write a brief job description.
7. Submit a project budget.  Include all sources and amounts of funding.
8. Submit a recent copy of the federal tax determination letter.

 Application deadline:  October 15, 2007

*Application is due by noon the following Monday if 15th is on a weekend.  Notification:  by January 31, 2008
